Jerry Chidester, MD

Dr. Chidester returns to Utah with his family of five after completing seven years of plastic and reconstructive surgery specialized training in Southern California, with an added emphasis on hand and upper extremity surgery from the renowed Joseph H. Boyes Hand Fellowship in Los Angeles. He has partnered with Dr. P. Daniel Ward and Form Medical Spa in Salt Lake City, and is excited to offer the full spectrum of plastic and reconstructive surgery, hand, and aesthetic surgery to his patients.
